Reason for CPAN Consultation: To receive medication and resource recommendations for a patient
Explanation of the Case and Outcome of the Call:
I had a patient who had just been released to his stepmother from a long mental health hospital stay with multiple diagnoses and multiple medications. The mother had many questions and was at her wits end trying to get answers and resources for her struggling child. She came to see me at our Tyler Care Clinic which serves as a safety net for all patients with a mission to provide culturally competent and sensitive care for our community, making healthcare accessible for all.
I immediately thought of CPAN and all the experts they have to help families just as this. I called. They connected me with a psychiatrist at the time most convenient for me. The psychiatrist was attentive, concerned, and most of all very helpful. She provided medication and resource recommendations to help this child who had been experiencing multiple serious side effects because of polypharmacy.
The help CPAN provided was lifesaving. The mother was in tears when I called back stating no one has ever seemed to care until now. She was thankful for the assistance and resources we provided, not only for this child but for the family dealing with this traumatic event.
